Metronidazole (Flagyl) is a commonly used antibacterial and antiprotozoal medicine. Adverse effects include vomiting, hepatotoxicity, neutropenia and neurological signs. Toxicity is seen at the time that the dose exceeds 60 mg/kg/set time for 3-14 days, although more dogs have become toxic at drop doses. It is commonly seen while the dose to treat Giardia is used longer than the 5-7 days that is recommended.

Metronidazole is liver metabolized such liver disease may increase metronidazole disposition levels and signs of toxicity have power to develop.

The neurological signs seen through this toxicity in dogs include morose generalized ataxia, vertical nystagmus, stiffness, tremors, superintendent tilt, and seizures. Cats can expand disorientation, cortical blindness, ataxia without nystagmus, seizures and seam knuckling.

Metronidazole binds to GABA receptors inhibiting the effects of benzodiazepines similar to flumazenil’s performance. Central vestibular axonopathy and purkinje small room loss occurs. CSF analysis may exist normal or show a mild be augmented in protein. Brainstem auditory evoked responses and CT/MRI are vertical. Drug levels may be measured.
Treatment involves unmediated cessation of the metronidazole and diazepam therapy. Diazepam exerts its antivertiginous furniture by facilitating the inhibitory neurotransmitter GABA not more than the vestibular system and shortens the retrieval time significantly (hours compared to days). The protocol with a view to diazepam administration is an initial IV dose of 0.5 mg/kg followed by 0.5 mg/kg PO q 8 hours in the place of 3 days. Prognosis with timely manipulation is excellent for full recovery.
